Three Star Ratings Measures Disappeared for 2027. If You Sold Against Them, So Did Your Pitch.

Oct 9, 2026

Three measures came out of the 2027 Medicare Advantage Star Ratings set: pain assessment in older adults, medication reconciliation following a hospital discharge, and medication therapy management reviews. Their removal was expected from prior rulemaking, which does not make the commercial effect smaller.

If part of a company’s value proposition to a health plan rested on performance against one of those measures, that part of the pitch stopped working this year. Not weakened. Stopped.

This is the mechanic worth understanding about selling into Star Ratings. The measure set is effectively the product roadmap of the buyer, and CMS rewrites it on its own schedule. A company that anchors its positioning to a specific named measure is renting its relevance from a federal rulemaking cycle it does not control.

The companies that handle this well do one thing differently. They sell the clinical outcome first, then map that outcome to whichever measures currently reward it. The product does not change and the story stays durable. What gets updated each year is the mapping, not the narrative.

This is also why the April 2026 restructure mattered more than it appeared to. Eliminating eleven administrative process measures did not just change how plans are scored. It repriced an entire category of vendor.

If a measure you depend on has just left the set, the work this quarter is not a new product. It is a new map.
